Entry Form - step 1

Welcome to the online Smokefreerockquest entry form. This is where your path to rock fame begins...

Before starting make sure you have:

• All your band members' names, phone numbers, postal addresses, emails, and the name of the school each of them are attending.
• A printer ready to print out your entry form.
• A photo and biography of your band (optional)

To enter, just fill in the next three pages, print off the PDFs at the end (they will be emailed to you too), get them signed by your parent/guardian and principal/HOD, and post them to the address provided on the form to confirm your entry.

If you don't have all the guff for your entry, don't worry, just create a login name, password and enter your email address below, fill in the info you do have, and click ‘save entry’ on the third page of the entry form. You’ll then be sent an email with your login details. Click the activation link, and you’ll be able to login and edit details, and submit your entry when you’ve filled out all the required fields.

Please note - you will NOT be entered into the competition until Smokefreerockquest has received all your signed entry forms and you have received your entry pack.
